What Is A MnCHOICES Assessment?

MnCHOICES is a comprehensive web-based application that integrates assessment and support planning for Minnesotans who need long-term services and supports. MnCHOICES is for people of all ages who have any type of disability or need for long-term services and supports. MnCHOICES embraces a person-centered approach to ensure services meet each person's strengths, goals, preferences and assessed needs.

  • It is an in-person visit with a MnCHOICES certified assessor to learn about your needs, goals and preferences.
  • It helps to determine what kinds of supports you might be able to receive, including public programs that might pay for those services.
  • It helps you learn about and access other support options if you do not qualify for publicly funded programs.
  • It usually occurs where you live.
  • It is free to you.
  • It will occur within 20 calendar days of your request.

MnCHOICES is for people of all ages who have any type of disability or need for long-term services and supports. MnCHOICES embraces a person-centered approach to ensure services meet each person's strengths, goals, preferences and assessed needs.

Who Can Have A MnCHOICES Assessment?

If you are a person of any age with a disability or in need of long-term services and supports, you may ask for a MnCHOICES assessment. You do not need to be eligible for Medical Assistance or any other publicly funded program to receive an assessment. However, to receive many available services, you will need to be eligible for Medical Assistance or another publicly funded program.
